Support Coordination
Support coordination enables you to optimise the utilisation of your planned supports. It is a form of support that focuses on enhancing your abilities in the following ways:
Understanding and leveraging your NDIS plan to pursue your objectives.
Connecting you with appropriate NDIS providers, community resources, mainstream services, and other government assistance.
Building your self-assurance and skills to effectively utilise and coordinate your support.
Support coordination services are typically provided by a support coordinator or a specialist support coordinator.
Your plan may include three levels of support coordination:
Support connection: This support aims to strengthen your capacity to engage with informal, community, and funded supports. Its purpose is to maximise the benefits of your plan and facilitate the achievement of your goals.
Support coordination: This support entails coordinating your various supports to enhance your understanding and utilisation of your plan. A support coordinator will collaborate with you to ensure a balanced mix of supports that enhance your ability to maintain relationships, manage service delivery tasks, live more independently, and participate actively in your community.
Specialist support coordination: This level of support coordination is designed for individuals facing complex circumstances and requiring specialised assistance. A specialist support coordinator will help you manage challenges within your support environment and ensure consistent service delivery.
