From lived experience to leading change in care

Evolving Pathways was born from lived experience — and a deep belief that care can, and should, be better.

As both a clinician and a parent of a child with multiple disabilities and complex health needs, I’ve lived on both sides of the system — the professional side that strives to help, and the parent side that feels exhausted, unheard, and overwhelmed. Living with multiple sclerosis myself has deepened my understanding of what it means when care doesn’t always feel caring.

I know what it’s like to juggle endless appointments, to advocate for the right support, and to still wish there was more humanity in the process. I’ve seen how the system can unintentionally wear people down — and I’ve felt the frustration of wanting care that truly connects.

Evolving Pathways was created to be the service I wished existed — a place where care is personal, communication is human, and families feel seen, supported, and understood.

Today, our team brings that same compassion and professionalism to every home and every individual we support — because everyone deserves care we’d be proud to give our own loved ones.

Our story began with a clear purpose: to redesign how care is delivered by focusing on people, not processes. We exist to remove barriers, replace frustration with understanding, and deliver services that combine clinical excellence with genuine compassion.

We provide complete health and social care across South Australia — supporting people of all ages and abilities, including NDIS participants, Aged Care and Privately Funded.

Clinical and Health Support

Our clinical team brings compassion and expertise into every home. We deliver high-quality, evidence-based care that helps individuals stay healthy, safe, and in control of their own lives.

• Specialist Nursing — wound care, catheter care, complex and palliative care delivered with dignity and respect.

• Medication Management & Health Monitoring — ensuring safety, stability, and confidence through skilled oversight.

• Continence Assessments & Care Planning — supporting comfort, dignity, and independence through personalised solutions.

• Allied Health Services — including Occupational Therapy, Psychology, Developmental Education, Counselling, and Allied Health Assistants — working together to promote functional independence, mental wellbeing, and quality of life.

Daily Living and Home Support

Our support team makes everyday life easier, safer, and more enjoyable — helping individuals maintain independence while living the way they choose.
We believe that the right support can transform not just a person’s day, but their entire sense of confidence, connection, and wellbeing.

• Personal Care — respectful, person-centred assistance with daily routines such as showering, dressing, and grooming — always delivered with dignity and empathy.

• Meal Preparation & Household Assistance — supporting nutrition, comfort, and independence through healthy meals and a helping hand around the home.

• Cleaning & Gardening Services — ensuring your home and surroundings stay fresh, organised, and reflective of your personal space and style.

• Supported Community Living & Respite Options — providing flexible in-home and short-term support so individuals and families can recharge, reconnect, and thrive.

• Social and Community Participation — from holiday clubs and day activities to creative workshops, crafting, and community outings, we help people stay connected and engaged with what brings them joy.

Therapy and Educational Groups

Our therapy and educational groups are designed to bring people together — to learn, grow, and connect in ways that build both skill and confidence.
We believe therapy should feel meaningful, engaging, and empowering — not clinical or cold. Every group is a safe space where individuals can explore, express, and evolve at their own pace.

• Skill-Building and Confidence — practical sessions that support communication, self-regulation, and everyday life skills in a fun, inclusive setting.

• Creative and Therapeutic Expression — group programs that use art, music, movement, and play to encourage emotional release, connection, and joy.

• Parent and Carer Education — workshops that empower families with strategies, understanding, and confidence to support their loved ones at home.

• Social and Peer Connection — opportunities for participants to share experiences, form friendships, and feel part of a community that understands them.

Our group programs are led by our Allied Health and Developmental Education teams and are tailored to the unique needs and goals of each participant. Whether it’s building emotional resilience, developing life skills, or simply feeling more connected, our focus is on creating spaces where people can thrive — not just cope.
