Founded by a clinician, parent of a child with complex disabilities, and a person living with multiple sclerosis, our service was built to address the real gaps families face — fragmented care, poor communication, and systems that prioritise process over people.
We deliver coordinated, respectful care that restores control to individuals and families.

Victori (Vikki)
I am a Nurse, Wife, and Mother with a genuine passion for delivering quality care in the comfort of people's own homes. Alongside my roles, I find joy in various activities such as camping, reading, art and craft, patchwork, and taking long walks. My journey as a caregiver is deeply personal as both my son and I have unique disabilities and specific needs. Through our own experiences, we have come to understand the crucial importance of finding reliable assistance and care within our community. Faced with difficulties in locating suitable support, we took matters into our own hands and established Evolving Pathways, an initiative aimed at assisting others like us in accessing high-quality care within our local community.

Chantel is a compassionate member of Evolving Pathways' Management team. With 16 years of experience in the hospitality industry, she found her passion for mentoring trainees with learning difficulties. Chantel's love for social science and neuroscience drives her to support others in living their best lives. As a certified Support Worker with an NDIS Worker Orientation certificate, she brings expertise and empathy to her role. Chantel's dedication and knowledge make her an invaluable asset to our team, positively impacting the lives of those she supports.
Cleaning ManagerKelly H
Kelly is an integral member of the Evolving Pathways management team.With over 25 years of experience in the disability sector, she brings a wealth of knowledge as a former early years and special education teacher. Kelly's journey at Evolving Pathways began as a support worker, where her dedication and exceptional skills quickly propelled her into the administrative and management team.
Kelly's expertise, combined with her passion for making a positive difference, greatly contributes to the success of our team and the individuals we serve. Her strong background in education and her NDIS Support Coordinator training enable her to navigate complex challenges with compassion and efficiency.
Support Worker & Education ManagerCaitlyn Farrugia
Caitlyn is a Developmental Educator who provides neuro-affirming, evidence-based support focused on building practical life skills and long-term independence. Her work supports individuals with social skills, emotional regulation, sensory needs, communication, executive functioning, daily living skills, and life transitions. She uses a person-centred, strengths-based approach that prioritises safety, trust, inclusion, and meaningful participation across clinic, home, school, and community settings.
Developmental EducatorGladys is a compassionate Occupational Therapist who supports children and adults to participate more fully in everyday life through practical, individualised strategies. With a Master of Occupational Therapy and a strong nursing background as a Registered Nurse, she brings clinical insight, holistic care, and strong communication skills to her work. Her experience includes cognitive assessment, sensory modulation, aged care support, and delivering collaborative, client-centred therapy across clinic, home, school, and community settings — helping individuals build independence, confidence, and meaningful participation in daily life.
Occupational TherapistGladys Chiu
Gillian is an experienced Occupational Therapist providing neuro-affirming, evidence-based support across paediatric and adult settings. She takes a holistic, person-centred approach to help individuals improve emotional regulation, social skills, cognitive function, independence, and recovery following neurological or mental health challenges. Her work spans clinic, home, school, and community environments, supporting clients through rehabilitation, everyday life skills, and meaningful participation.
Occupational TherapistGillian Cooke
Kim supports individuals aged 3 to adults with emotional regulation, executive functioning, social skills, sensory regulation, daily living skills, community participation, and Functional Capacity Assessments. She brings over 20 years of experience in education, teaching, and inclusive leadership, with postgraduate qualifications in Autism & Neurodivergent Studies and Secondary Teaching. Her neuro-affirming, strengths-based practice focuses on building confidence, independence, and practical life skills through collaborative support delivered across clinic, home, school, and community settings.

Sarah brings a strong background in occupational therapy and secondary education, supporting children aged 5+ to develop emotional regulation, executive functioning, and practical daily living skills. Her work spans clinic, school, home, and community settings, where she delivers neuro-affirming, trauma-informed therapy that helps young people build independence, confidence, and meaningful participation in everyday life.

The nursing team at Evolving Pathways have helped us immensely with my partner. Without them, my partner would be in a nursing home by now which he didn’t want. We all want him to be able to have his wish to die at home, pain free with us by his side. Evolving Pathways are helping us deliver his wish. Evolving pathways support me three to four times a week at home to do my leg dressings. I have the same two nurses and they are wonderful and caring.

My two kids attend horse riding twice a week with one of the workers from Evolving Pathways. We have the same carer every time and the kids love her. My son is getting to an age where it’s not cool to have mum do things with or go out and about with. He has Autism and wants to go and do things like other kids his age. We have one of Evolving Pathways team members visit weekly to take him out and about. He loves it and its great he gets a male role model at the same time.
